We are excited to announce that Equality Care Center is transitioning its business to Feliz Care Center on September 1, 2023. The same great care that you received from Equality Care Center will continue with Feliz Care Center. The location remains the same, 3306 W. Roosevelt St., Phoenix, and most providers will continue with Feliz Care Center.
If you have an appointment scheduled with your Equality Care Center provider, it will be transitioned to the Feliz Care Center system and you do not need to do anything at this time.

If you would like to transition your care to another practice or would like a copy of your medical records after September 1, 2023, please contact Korina Olivas or Sandra Valencia at the Feliz Care Center (480) 659-9696.
